Plan the first meals before leaving hospital
Returning home can leave little energy for shopping or cooking. Discuss meals with the hospital's social team before discharge and identify who will receive the first delivery. Food instructions from the treating team should guide the provider discussion. A meal round does not replace a prescribed plan or personal help needed to eat safely and comfortably.
Check when temporary assistance ends
Some pension schemes or assistance contracts consider support after hospitalisation, subject to their own conditions. Contact the relevant body before incurring costs where prior authorisation is required. Record the end date and plan a follow-on arrangement if difficulties continue. The social team can help coordinate food deliveries, home assistance and the related applications.
